The U.S. Centers for Disease Control and Prevention released a statement on the latest Morbidity and Mortality Report which focused on prevalence of diabetes among adult Americans, "a person should know that they can prevent the disease or delay it by getting more exercise, adjusting their diet, and losing weight".

OPTING-IN TO THE 2017/18 WELLNESS PLAN OPTION
What are the 3 requirements needed to be eligible for the 2017/18 Wellness Plan Option?
- Complete the on-line Personal Health Assessment questionnaire.
- Complete a biometric health screening either at a District or Saint Alphonusus location, OR with your own physician.
- Complete 1 approved Wellness Activity focused on maintaining
or improving health.
The ACTIVITY options will be met by completing either a WellSteps campaign, WellSteps ACTIVITY TRACKER entries, or by submitting a completed Wellness Activity Form which verifies participation in a personal Wellness Activity. The deadline date for completing all 3 requirements was Friday, March 3, 2017.

If one prefers to use a personal healthcare provider to meet the biometric health screening requirement, fax a completed Health Screening Verification Form or Exemption From a Health Screening Form to WellSteps. Deadline date was Friday, March 3, 2017.
